As other commenter mentioned, you can use pg_repack. We also had similar issue and we configured it to run every month and it works flawlessly.

No. I work for a Fortune 100, we use Quarkus for lot projects, the dev reload is amazing, it just takes less than a second to reflect the change. It fast enough that I feel like I work with a Python project.

I guess its depends, for many use cases, it can be managed at application level. I've parted ways with FK for a long time since it created more hassles than it solved esp when it comes to sharding and replications.
