   I understand why this is the way it is (cost-savings), but this needs to change. It has been shown that preventative screening can be very cost effective as opposed to reactive treatment.

   In some countries, you get paid to publish in scientific journals. It is a way for the country to get recognition. There is massive incentive then to publish, apart from career incentives.
