Another excellent C++ book that does this "the right way around" is "A C++ Primer" by Lippman, Lajoie & Moo. The 5th edition is rewritten for C++11. Disclaimer: I've only used the 4th edition.

I'd assume the rules are the same in principle but you would be more likely to be considered autonomous in practice if audited. Also because you're probably providing your own equipment, office etc. I actually did this for a while

We've been using their VPSes for a couple of months and are happy with them so far, even if the management interface is a bit clunky. They also have a good selection of data centers if location/latency is important.
