If dynamic refactoring so easy they were doing it 30 years ago, but one of the best IDE makers in the world can't generalize it for the masses today, doesn't that tell you something?

I agree with this point. You are using a more flexible stubbing framework and THAT is what the point of this post should be about. The rest is just semantics. My questions with TDD always come with the refactoring.

Why not just use Crucible if you are running a small startup? There license cost is $10 right now, and it has a lot of niceties. http://www.atlassian.com/software/crucible/
