They have to choose the default tip amounts so I’m pretty sure tips are off by default. Also, there was an interview on Vox or NBC where business owners state it’s off by default but “you are leaving money on the table by not asking.”

Nobody expects it come without a cost. But I would sure as hell prefer paying roughly the same in taxes as what my premium is in order to not have to worry about leaving the doctor or hospital with a lifetime of debt.

I’m not sure it’s that unrealistic. The trailer shows off a LOT of features carrying over from CS1. Some stuff might be cut, sure, but they aren’t swapping engines so the breaking changes might not be as significant as one might expect.
