They walk everywhere. That is, they have a transit system that allows them to walk (and bike) everywhere. That combines with smaller portions and strong social pressure to remain trim.
Couple caveats:
1) There is a tradeoff in lower average body size/height, which doesn't seem to bother them as a whole, but which any individual athlete/model/actor/businessman might curse.
2) I know of several Japanese men in the 240-400 pound range. Many of them seem to enjoy being larger, but for those who don't, being Japanese in Japan wasn't a silver bullet.
