What makes me kind of angry is this is a totally and easily fixable problem but for whatever reason, the people who could do something about it have no desire for improvement and would happily let the cost and inconvenience continue. Not one single person in that system thinks the very obvious "A name without any other data is worthless, let's just delete it from the list". If people lack agency then you already have an ineffective system but my guess would be that no-one person has the authority to take executive action and every change however daft takes 30 people in a meeting discussing it for an hour so it just gets left as it is.
