Mt. Gox offers two-factor authentication methods (Yubikey and Google Authenticator). Neither was used. Lastpass offers two-factor authentication methods (Yubikey and Google Authenticator). Neither was used. But also, nobody who also works with Intersango had access to either of those two accounts.
